The pre-screen process is designed to gather all the clinical and administrative groundwork required for a safe and efficient consultation. By collecting this data upfront, the pharmacist can focus entirely on clinical decision-making during the appointment.
Clinical & Personal Information
The pre-screen captures the following essential data points:
- Address: Confirms the patient's residency details for clinical records.
- DOB and Sex: Vital for ensuring the patient meets the age criteria for the specific PGD or service.
- GP Surgery: Captures the patient’s registered doctor to facilitate integrated care.
- Current Medications: A list of any drugs the patient is currently taking to check for potential contraindications or interactions.
- Medical History: Significant past or present medical conditions.
- Health Concern: A brief description of why the patient is seeking the service today.
- Allergies: Critical safety information regarding drug or environmental allergies.

Consents & Authorisation
The final stage of the pre-screen is the Consent Module. This section is legally required and ensures the patient is fully informed before the consultation begins.
Patients are asked to make specific choices regarding:
- GP Data Sharing: Whether they authorise the pharmacy to share the details of this consultation with their registered GP surgery.
- Marketing Consent: Whether they wish to receive future communications or offers from the pharmacy.
- Signature: A digital signature to verify the information provided is accurate and that they consent to the service.
⚠️ Important Note for Staff: > Even if a pharmacist or staff member has assisted the patient by typing their answers for the clinical section, the patient must complete the Consent & Signature stage themselves. You must hand the device to the patient at this point to ensure their legal authorisation is captured directly.
